What to bring to
your Appointment.

Records
We are able to offer low evaluation
fees to patients by acting as consultants & not as primary health care
providers. Meaning the problems you suffer from have been diagnosed
already.
To document this:
Bring any recent
records of What your ailment is (your
diagnosis);
Progress notes, prescriptions, discharge summaries from
hospitals all are helpful. Short letters from your Health Care
professional are extremely helpful if they state your diagnosis and how
long they have been treating you for it. If you are a current prop215
patient bring your prior recommendations.
If you have not seen a doctor or
health care provider for a long time and have no documentation, The
physician may still evaluate your need for a recommendation.
However he may give
you
a temporary recommendation and need to refer you to an outside primary
care physician or request a follow-up visit at the clinic. Followup visits
are free at AMC for one year after initial consultation, For Safety
and Efficacy reasons.
For
patients that are Underinsured.
Another option is to visit a free
clinic in your community prior to your appointment.
